Soft tissue defects of hand is a common problem due to machinery injury or RTA. Coverage of these wounds is a priority for proper functioning of hand. Distant flaps ( groin flap, abdominal flap and PUPF) are more commonly used to cover these defects due to easy harvesting technique and reliable vascularity. The problem in distant flap is that it is a two stage procedure and requires more hospital stay. The posterior interosseus artery( PIA) flap is a good choice as it is a single stage procedure. This procedure reduces hospital stay and allows the patient to return to work quickly. The flap dissection can be challenging and requires meticulous skill. Appropriate planning is needed for successful outcome.
