Create an input form and access it with a button. Excel VBA.

In the VBA editor, under the menu item "Insert" select "UserForm", it will be created automatically.

Switch to the worksheet and activate design mode. Double-click the "Mask" button with the left mouse button. Enter the code in the opened subroutine (Sub).

Switch to the worksheet and deactivate design mode. The form will then appear.
