The Single Leg Calf Raise is a simple but powerful test and exercise used to assess and build calf strength, Achilles tendon capacity, and foot and ankle control. It’s one of the most reliable ways to gauge whether someone is ready to return to running or tolerate higher loads through the lower limb.
It’s commonly used in Achilles tendinopathy rehab, following Achilles rupture, for foot and ankle injuries, and as part of ACL reconstruction testing protocols. I often use it when assessing strength capacity before athletes progress to impact or high-speed change of direction work.
In general, I look for around 25 full range, controlled reps as a return-to-running target, and 30 before return to play, but the specific rep goal depends on the full clinical picture and injury history.
Cues to perform it properly:
-Use a 1:1 tempo (one second up, one second down) – a metronome helps keep it honest
-Work through full range of motion – lift as high as possible each rep
-Keep the knee straight throughout
-Use only minimal fingertip support for balance – no pushing off
Why runners should be doing this
If you're a runner, this is one of the best ways to benchmark your ankle function. Research shows the calf–Achilles complex absorbs and generates up to 6 to 8 times your bodyweight with every step during running (Dorn et al., 2012 – PubMed ID: 23125141).
If you can’t meet this demand, you’re far more likely to develop issues like Achilles tendinopathy, calf strains, or overload-related foot and ankle pain.
This test helps expose any gaps in strength, endurance, or side-to-side asymmetry – all of which matter when it comes to staying injury-free and performing well.
