Medicare Social Determinants of Health (SDoH) Assessment: CPT Code, Billing, and Reimbursement

In this video, we'll discuss the new 2024 CPT code G0136 for the assessment, including billing requirements and reimbursement rates. You'll learn about the SDoH assessment as well as who can administer it. You'll also learn about the benefits of administering the assessment during an Annual Wellness Visit (AWV), as if a Social Determinants of Health Assessment is done during an AWV, it is completely covered for patients under Medicare Part B. An SDoH assessment can lead to enrollment in the new Community Health Integration (CHI) program as providing an SDoH assessment is strongly recommended before enrolling. An SDoH Assessment is a valuable tool to help to identify known or suspected SDoH needs that may interfere with the diagnosis or treatment of a patient and can help lead to improved outcomes.
