On June 10th 1990 British Airways Flight 5390 took off from Birmingham heading to Malaga.
81 passengers on board.
30 minutes after takeoff the cockpit windscreen exploded at 17,300 feet.
Captain Tim Lancaster was sucked headfirst out of the aircraft.
His knees caught on the flight controls - the only thing stopping him disappearing completely.
Flight attendant Nigel Ogden grabbed his legs instantly.
Outside - minus 17 degrees. 200 mile per hour winds.
Lancaster's head repeatedly slamming against the fuselage.
The crew believed he was already dead.
They held on anyway for 20 minutes while the co-pilot made an emergency landing at Southampton.
Lancaster was pulled back inside alive.
Frostbite. Fractures. Shock.
He was back flying within five months.
The investigation found the windscreen had been fitted with bolts slightly too small.
81 people almost died over the wrong size bolt.
