Paul Ehrlich developed methods to stain cells, standardize passive immunization, Chemotherapy to treat diseases, and laid the foundation for modern medical practices. Paul Ehrlich among others developed innate and adaptive immunity. His work opened up the window for immune oncology. Till 1948 it was believed that cancer is incurable. The Discovery of anti-folate (Methotrexate) by Dr. Yellapragada SubbaRow, Director of R&D at Lederle changed the way we started treating cancer. Understanding the pathology of cancer cells and tumor tissues and their microenvironment opened up a better approach to understanding cancer progression from benign to malignant. Advances in DNA recombinant Technology and Monoclonal antibodies in the 1970s enabled a better understanding of types of cancer cells. The Discovery of p53 in the late 1970s gave a better view of checkpoints in the progression of cancer.
In the 1990s discovery of checkpoint inhibitors (anti-CTLA-4 and anti PD(L)-1) and approvals of ipilimumab and pembrolizumab opened up a new approach to cancer therapy. The current status of 7 anti PD(L)-1 approved checkpoint inhibitors, improvements in response rates, overall survival, tumor mutation burden, emerging adverse events will be presented. Few newly identified targets (e.g., Adenosine axis pathway i.e., anti CD39, anti CD73, and anti-A2aR/A2bR inhibitors), translation medicine efforts to select patients based on biomarkers and checkpoint inhibitors will be discussed.
