ACANTHAMOEBA KERATITIS
Acanthamoeba is free lying amoeba seen in soil, fresh water, sewage
Seen in contact lens wearers & contamination or inoculation with soil or muddy water or trauma with organic matter
C/F
Initial photophobia to excessive pain in established infection
Pseudodendrites
Ring of Wessley
Radial Keratoneuritis & Stromal satellite infiltrates
Ring abscess with hypopyon and necrosis
Can mimic fungal corneal ulcer
Ix
High deal of suspicion in non-responding ulcers
Confocal Microscopy
Transparent colonies are seen in Non nutrient agar enriched with e coli
Rx
PolyHexaMethyleneBiguanide (DOC) & Pentamidine is indicated in treatment.
Long treatment required
Penetrating Keratoplasty in non-responding cases
