Puffy Nipples in Men Explained | Glandular Gynecomastia, Causes & Permanent Treatment
In this video, Dr. Ankit Gupta, Plastic Surgeon at Panache Aesthetics, explains a very common yet rarely discussed problem in men — Puffy Nipples, also known as Glandular Gynecomastia.
Many men notice a dome-shaped bulge around the nipple area, especially while wearing tight T-shirts. Although the rest of the chest may look fit and well-shaped, the nipple area protrudes, making men feel embarrassed, conscious, or uncomfortable in public. Because of shyness and lack of awareness, most people avoid talking about it — even though the condition is very common and completely treatable.
What Are Puffy Nipples?
Puffy nipples are usually caused by a small breast gland located just beneath the nipple and areola (the darker area around the nipple). This is not fat. It is a true glandular tissue, which pushes the nipple outward, giving it a rounded or puffy appearance.
This gland becomes more noticeable during:
Cold or hot temperatures
Emotional or physical arousal
Wearing fitted or tight clothing
That is why the nipple suddenly appears more prominent even if the chest otherwise looks normal.
Why Do Puffy Nipples Occur?
Puffy nipples can develop due to several reasons:
Hormonal imbalance
Puberty-related changes (commonly around age 13–15)
Massive weight gain or weight loss
Side effects of certain medicines
Use of anabolic steroids
In many cases, the condition persists even after puberty and does not resolve on its own.
How to Know If It’s Gland or Fat?
A simple way to differentiate is by gently holding the nipple area:
If it feels rubbery or firm, it is a gland
If it feels soft and spread out, it may be fat
Exercise and gym workouts can reduce fat — but they cannot remove a gland. This is why many lean, muscular men still struggle with puffy nipples despite intense training.
Real Patient Case Explained
In this video, a real patient case is shown. The patient had even been selected for the army, but during medical evaluation, his puffy nipples were identified as gynecomastia and he was advised to correct it. Despite extensive gym training and achieving a very lean physique, the problem remained — because it was glandular, not fat.
Permanent Treatment: Gland Excision Surgery
The definitive treatment for puffy nipples is surgical removal of the gland. In this case:
Surgery was done under local anesthesia
A small C-shaped incision (~1 inch) was placed just below the areola
The incision is hidden at the natural color junction, making the scar almost invisible
The entire gland was removed completely
The surgery takes 30–45 minutes and is almost pain-free except for the initial anesthesia injection.
Recovery & Aftercare
Stitches are removed or dissolve on their own in about 10 days
A pressure garment is advised for around 2 weeks
Gym and heavy exercise should be avoided for 1–2 weeks
Cardio can usually be started earlier with garment support
Results & Permanency
Within just 2 days, the chest already looks flat and natural. Once the gland is removed, the results are permanent, and recurrence does not occur.

• Can exercise fix puffy nipples?
No. Exercise reduces fat, not gland.
• Is the scar visible?
No. After 1–1.5 months, it is extremely difficult to locate. After 6 months, it is almost invisible.
• Is the surgery painful?
Only the anesthesia injection causes brief discomfort. Otherwise, pain is minimal.
• When can I go back to the gym?
