Obesitas verhoogt de kans op het ontwikkelen van chronische ziekten, zoals hart- en vaatziekten, lever- en nierziekten en type 2 diabetes. Verschillen tussen personen in de oorzaken en gevolgen van obesitas, evenals biologische adaptaties die bijdragen aan gewichtstoename na afvallen, bemoeilijken de preventie en behandeling van obesitas en gerelateerde gezondheidsproblemen. Door middel van translationeel onderzoek probeert mijn team daarom de biologische mechanismen te ontrafelen die ten grondslag liggen aan het ontstaan van cardiometabole complicaties en de respons op (leefstijl)interventies, en de verschillen die hierin bestaan tussen mensen met obesitas.

In mijn inaugurele lezing leg ik uit waarom we bij de diagnose en behandeling van obesitas verder moeten kijken dan de kilo’s, verder moeten kijken dan de Body Mass Index (BMI). Juist de vetverdeling en de mate waarin het vetweefsel nog goed functioneert zijn van invloed op de suiker- en vetstofwisseling en chronische laaggradige ontsteking in mensen met obesitas, en bepalen daarmee in sterke mate het risico op cardiometabole ziekten. Door een beter begrip van onderliggende biologische verstoringen, die kunnen verschillen tussen personen met obesitas, kunnen we effectievere preventie- en behandelstrategieën ontwikkelen die zijn toegespitst op bepaalde subgroepen van de populatie.

Obesity acts as a gateway to many other chronic diseases, including cardiovascular disease, liver and kidney diseases, and type 2 diabetes. Inter-individual variation in the causes and consequences of obesity, as well as biological adaptations underlying weight regain after initial weight loss, complicate the prevention and treatment of obesity and related health problems. Using a translational research approach, my team therefore investigates the biological mechanisms that underlie the development of cardiometabolic complications and the response to (lifestyle) interventions, and the differences that exist between individuals with obesity.

In my inaugural lecture, I explain why we must move beyond body weight, beyond the Body Mass Index (BMI), when diagnosing and treating obesity. Body fat distribution and the extent to which the adipose tissue still functions properly impacts glucose and lipid metabolism, as well as chronic low-grade inflammation, in people with obesity, and therefore largely determines the risk of cardiometabolic diseases. A better understanding of the underlying biological impairments, which may differ between individuals with obesity, will contribute to the development of more effective prevention and treatment strategies, tailored to specific subgroups of the population.
