Who qualifies?
Everyone who
has been living in the Czech Republic legally for long enough. Long enough usually means 5 years but it can be as little as 2 years or as long as 10 years.
has been making enough money on monthly basis - the required monthly level is calculated based on how many people live in your household,
speak good enough Czech - A1 until 30.8.2021, A2 from 1.9.2021 on.

What benefits does the Permanent Residence Permit have?
no one will ever kick you out from CZE,
status is unlimited, you only need to renew the card,
free access to the Czech Labor market,
access to public health insurance,
not tied to a purpose of stay,
voting rights,
required step to getting Czech citizenship,
easier to move within the EU.

What documents are needed?
application form,
passport + visa/residence permit,
1 passport sized photo,
proof of accommodation,
proof of income,
Czech language certificate,
2 500 CZK admin fee - paid in revenue stamps ("kolky").

What you should be careful about?
permanent residence application does not grant you bridge visa,
not all stays outside of the Czech Republic are counted towards the Permanent Residence Permit.
