NASA has released a scathing 300-page investigation report into the 2024 Boeing Starliner test flight — officially declaring it a Type A Mishap, the agency’s most serious classification.
Astronauts Sunita Williams and Butch Wilmore were originally scheduled for an 8–14 day mission but ended up stranded in space for over nine months before safely returning in March 2025.
NASA Administrator Jared Isaacman admitted:
"We returned the crew safely, but the path we took did not reflect NASA at its best."
The investigation revealed:
• Loss of six degrees of freedom during ISS docking
• Thruster malfunctions and helium leaks
• Hardware failures and qualification gaps
• Leadership missteps that posed unacceptable risks
The report sharply criticized Boeing’s “design and engineering compromises” and also faulted previous NASA leadership for failing to intervene sooner.
NASA has confirmed it will not fly another crew aboard Starliner until all issues are fully resolved.
The findings raise serious questions about the future of Boeing’s role in NASA’s Commercial Crew Program — and about how close this mission came to disaster.
