COVID 19 and Life Threatening Cardiac and Cerebellar Thromboembolic Events in Cardiac Operations

There are concerns surrounding false negative upper respiratory tract COVID 19 swabs in asymptomatic patients, planned for surgery. Here we present two patients who underwent major elective cardiac surgery. They were asymptomatic prior to the operation and had negative upper respiratory tract COVID 19 swabs. SARS-CoV-2 RNA was detected in postoperative Broncho-alveolar Lavage by PCR. They remained stable during the initial postoperative period, then developed major life-threatening thromboembolic events. The release of cytokines during cardiopulmonary bypass with COVID 19 associated thrombogenic factors can amplify thromboembolism. The question arises whether bronchioalveolar lavage to test for SARS-CoV-2 immediately post intubation should be routinely carried out, even in patients with recent negative swabs? If the result is positive and available prior to the operation, the surgeons can postpone non-urgent/nonemergency operations. However, if the result is obtained during/after the operation, the aim should be to keep hypo-coagulable status during the immediate postoperative period.
