Across the country, older adults are experiencing preventable falls, confusion, hospitalizations, and decline. Many of these challenges stem from complex medication combinations that may no longer be needed or may interact in ways that create new problems. In this talk, Dr. Canterbury shares how a collaborative and science driven approach to deprescribing empowers caregivers, supports clinicians, and restores safety and dignity for our most vulnerable communities. It is an invitation to create more clarity and stronger partnerships in care.
uDr. DeLon Canterbury, PharmD, BCGP, is President/CEO of GeriatRx, Inc. As a Board-Certified Geriatric Pharmacist, Dr. Canterbury has a vision to reduce over medication of senior adults by implementing medication action plans and drug screenings for geriatric patients in cooperation with patient’s providers. This novel approach of instituting personalized care improves patient satisfaction and wellness. He acknowledges that unsafe prescription medication practices significantly contribute to the cost of health care in an amount that exceeds $20 billion dollars annually. His efforts in executing Deprescribing protocols have saved families millions of dollars by preventing involuntary admissions to memory care facilities. Founder of the Deprescribing Accelerator, Dr. Canterbury trains prescribers, nurses and pharmacists in this innovative approach to holistic wellness for older adults. He envisions all pharmacists and senior care providers as “Deprescribing Advocates”.
