Tetanus has a 100% mortality rate in unvaccinated people who do not get ICU-level care.

It attacks the motor nerves and climbs toward the central nervous system. Early signs include muscle spasms, lockjaw, facial grimacing, a stiff neck, and back arching so strong it can break bones.

The dangerous spasms happen in the throat, causing difficulty swallowing, fear of water, laryngeal spasms that seal the airway, and breathing spasms that prevent you from breathing on your own.

A classic bedside sign is the spatula test. When you touch the back of the throat with a tongue depressor, a tetanus patient bites down instead of gagging.
