🦴 How Bone Infection Leads to Necrosis (Death of Tissue)
Bone infection, also called osteomyelitis, can progress to necrosis (death of bone tissue) if not controlled. This happens because infection disrupts the delicate balance of blood supply, immune defense, and bone metabolism.
🔎 Mechanisms of Necrosis in Bone Infection
Bacterial Invasion
Pathogens (commonly Staphylococcus aureus) enter bone via bloodstream, trauma, or surgery.
They multiply within the medullary cavity and Haversian canals.
Inflammatory Response
The immune system sends neutrophils and macrophages.
Pus forms inside rigid bone structures → increased pressure.
Vascular Compromise
Rising intraosseous pressure compresses blood vessels.
Reduced blood flow → ischemia (lack of oxygen and nutrients).
Bone Necrosis
Ischemia causes death of osteocytes and bone marrow cells.
Dead bone fragments (called sequestra) form, which harbor bacteria and perpetuate infection.
Chronic Cycle
Sequestra act as a nidus for persistent infection.
Surrounding bone tries to wall off infection by forming new bone (involucrum).
This cycle leads to chronic osteomyelitis with recurrent abscesses and sinus tracts
