Today’s pearl: Serum thyroid hormones don’t always reflect intracellular action, especially with inflammation.
During illness/inflammation, local deiodinases shift inside tissues: in acute skeletal muscle, D2 (“gas”) rises and D3 (“brake”) falls (↑ intracellular T3), while liver/adipose can show the opposite.
During chronic states, patterns vary by tissue.
Translation: tissue thyroid action ≠ serum T3/T4/rT3, so labs may not match symptoms.
