As of the end of 2024 there are no studies examining women with ADHD before, during, and after their perimenopausal years. This reflects how medicine in general has relatively ignored women, ignored the importance of menopause, and not recognized the importance of ADHD in women.
Nevertheless, research indicates that many women experience concentration, attention, memory and mood regulation issues during perimenopause (when estrogen and progesterone doses are fluctuating) and in menopause (where levels of these two hormones are minimal). Robust concentrations of estrogen receptors in the prefrontal cortex, involved in executive functions, are thought to be how menopause may impact brain function. The impairment of many women during perimenopause appears to be as severe as those who have full blown ADHD.
Preliminary evidence suggests that diminished estrogen levels result in less dopamine and more glutamate in the prefrontal cortex, and that treatments including hormonal replacement therapy, atomoxetine, and lisdexamfetamine can help reduce ADHD symptoms.
