Welcome to The Caring Neurologist, where clarity, compassion, and hope meet in the journey through Parkinson's care. In today’s episode, Dr. Sandeep Thakkar takes us deep into a topic that troubles many listeners: slowed cognitive processing, often described as brain fog or mental fatigue. Drawing from real clinic conversations, Dr. Thakkar breaks down the science behind bradyphrenia, the slowness of thought seen in Parkinson’s and explores why it’s often mistaken for memory loss or dementia.
You’ll hear why mental exhaustion happens even when memory seems intact, what’s really going on in the brain with dopamine and other key chemicals, and how factors like sleep, medication timing, and stress play major roles. The episode offers practical advice for optimizing clarity from tailored medication management to lifestyle changes and shares crucial insights for both patients and caregivers. If you’ve ever wondered why your thinking feels slow, or how to best support mental stamina, this episode is packed with guidance, reassurance, and the tools you need to navigate brain fog with confidence.

"In Parkinson's, a lack of dopamine makes that gate sticky. So it's just as it's hard for the brain to signal the legs to walk, like which we call bradykinesia, it becomes hard for the brain to signal thoughts as well and to kind of get the words out from the mouth."

"it's really not a hard drive problem or a memory storage problem like you see in Alzheimer's."

"But when people are in their on state, they're just right and the levels are just smooth and it's not peaking fast or dropping off fast, then the processing speed is a lot faster and attention's a lot more improved."

"So you want to think of the brain as having literally a plumbing system, which is the glymphatic system. And while you're sleeping, specifically during deep non-REM sleep, your brain cells actually shrink slightly. And they allow for the CSF fluid to flush out the metabolic waste. And with Parkinson's especially, one of the main wastes being flushed out is the alpha-synuclein, the toxic protein that causes the Parkinson's. So if you don't get enough sleep, especially deep sleep, that trash isn't picked up, and then you wake up with your brain feeling clouded because there's still metabolic waste, abnormal protein buildup, other natural neurotransmitters that haven't really been wiped away and cleaned up, and it makes like you feel like you're wading in your own swamp, right?"

"But BDNF is a fertilizer for the brain cells."

"There's data that says that it improves motor learning and structural brain connectivity for the long term with resistant weights. And then tai chi and yoga, you know what that does? It helps mental stamina, actually. So it reduces stress and cortisol levels."
— Dr. Sandeep Thakkar [00:36:00 → 00:36:14]
"Caring for someone with Parkinson's often feels like you're managing two different conditions, the visible motor symptoms, the tremors, the stiffness, and then also those invisible cognitive ones."
— Dr. Sandeep Thakkar [00:37:11 → 00:37:20]
"Parkinson's is generally a slow-moving disease. You know, you shouldn't see a drastic change over hours or days. And so that's when you have to think of something called delirium, a sudden intense confusion or disorientation."
