We want to be quite clear that this is guidance we should not have to write. ED crowding is not inevitable. It represents a fundamental failure in policymaking and in health service leadership and management.

Responsibility and accountability for fixing this problem lies with politicians, the civil servants who advise them and who implement the policies, and in senior health service leaders across the four nations and in health systems.

EDs and RCEM cannot fix this problem. Our members have to live with the consequences of these failures on a daily basis. They're trying to reduce the risks to their patients and also deliver the best care they can under
very difficult circumstances. This guidance is designed to support them as they try to do this.

It's also designed to provide advice for operational management teams within emergency departments, hospitals and local health systems about the best way to approach this difficult problem.

Finally, there's a summary of some of the evidence around crowding and why we think investment is best targeted by those who are able to enact the sort of change that is needed to make this problem go away.
