Throwers shoulder is a common problem faced by many young athletes. Here are a few FAQ's on Internal Impingement syndrome:
Q: What is Internal Impingement syndrome/ Thrower shoulder ?
A: Shoulder comprises of a ball (Humeral head) moving in a socket (Glenoid). Muscles of the shoulder joint make the ball move in the socket. When one of these shoulder muscle (Supraspinatus) gets pinched between the ball and the socket, typically in throwing position, it is called as impingement syndrome.
Q: What are the symptoms?
A: Severe pain while doing any weight training exercises using the shoulder joint (Typically bench press, Inclined bench press, cable fly, Shoulder press and front raise). There is sudden loss of strength while going to the extreme of movement. Night pains & Early morning stiffness in the involved shoulder. Painful restricted range of movement.
Q: How to diagnose?
A: Consult your sports medicine expert/ Orthopedic surgeon early. Any exercises leading to an athletic position of shoulder (ABduction & External Rotation) like in throwing of a ball, leads to severe symptoms and can be elicited every time the activity is performed. Night symptoms in young may also be an indication of Internal impingement syndrome.
Q: How to treat?
A: Consult your orthopaedic specialist. Treatment has few simple steps one can do at home. They include:
1. Rest to affected part
2. Abstain from activities that increase pain
3. Do not lift
4. Ice packs
5. Physiotherapy sessions
6. Anti-inflammatory/ Analgesic use as prescribed
7. Rehabilitation after pain/ symptoms start settling down
Q: What are the treatment options ?
A: Treatment may be in form of Conservative or Surgical Management. Most young patients with short course of disease respond very well to rest, ice packs, physiotherapy and rehabilitation. A small subset may require investigations like Skiagram, MRI and or other modalities to ascertain the exact diagnosis. Patients who fail to respond to conservative management may also require surgical procedure for resolution of symptoms.
Q: How to prevent Internal Impingement ?
A: Simple ways to prevent shoulder pain include:
1. Avoid heavy weight exercises
2. Proper form of exercise is important
3. Do not compare with others
4. Take rest days
5. Never work out in pain
6. Know your elastic limits
7. Proper gear
8. Proper/ Adequate nutrition
9. Avoid exercises when suspected injury
10. Consult the experts for help !
11. Avoid self (देसी) treatments.
