The following is the cockpit voice recorder (black box) transcript from the September 8, 1994 crash of USAir Flight 427 that took off from Chicago O'Hare airport intended for Palm Beach International in Florida with a stopover at Pittsburgh International.
While on approach at an altitude of 6000 feet Flight 427 entered the wake turbulence of a Delta airlines flight that was 4.1 miles ahead of them. The plane immediately began to bank and roll to the left and the aircraft stalled. The plane was pitched 80 degrees nose down and banked 60 degrees to the left when it hit the ground at 300mph only 28 seconds after flying into the wake turbulence.
All 132 souls on board were lost, 127 passengers and 5 crew, including Captain Peter Germano and First Officer Charles "Chuck" Emmett.
After a four and a half year investigation, the longest in NTSB history, it was ruled mechanical failure of the rudder caused the crash, which initially the FAA did not agree with. However after their own special investigation detected potential rudder failures in the 737 they wanted Boeing to redesign the rudder system.
Because of Flight 427 the 737 rudder system became safer, pilots received training on how to deal with a full rudder deflection, four additional channels of information were added to the flight data recorder to capture rudder commands, and Congress passed the Aviation Disaster Family Assistance Act which helps the families of those who were lost in a plane crash.
