Once, Twice, Three Times Malady is unlocked by clearing the Starcraft 2 Heart of the Swarm "Infested" mission before the platform is gassed for a third time.
This is a tight timer. You need to be very aggressive with Kerrigan right from the start and use her to clear most of the map. You will want to bring a few Infestors for Fungal Growth as well as to mind control Thors. This will help speed things along significantly.
Meanwhile, start building up some units at your base. I like to use Abominations, but really anything will work. Avoid clearing the left-hand side of the map.
