2. Deposit/pay the appropriate amount of Registration Fee to any of the following:

3. Fill-out the required information. Attach one (1) PDF file only containing the following documents in the google form:
a. Proof of payment
Must include the complete destination account number and date and time.
For Gcash or Paymaya transactions- screenshot of proof of payment with Reference Number.
b. Either of the following:
COLLEGE GRADUATE - Photocopy of Transcript of Records OR Diploma to prove that the applicant is a business course graduate (not required on PACTT members who will only attend the seminar).
COLLEGE UNDERGRADUATE - a Certification issued by the Dean / Dept. Chair / Registrar indicating that you have passed a 3-unit taxation subject.
CPAs - copy of PRC ID
PACTT MEMBERS - Copy of CTT or DIT ID card

4. Upon successfully filling-in the google form, kindly wait while we process your application. If there is a problem on your application, you will be contacted by the office.
5. Further instructions relative to and before the start of the review/seminar will be sent thru e-mail or private fb group.
