Epilepsy surgeries are performed when anti-seizure medications fail to control or significantly reduce seizures. This video provides a simple explanation of how epilepsy surgery works, the tests involved, and what patients can expect during recovery.
Before surgery, patients may undergo MRI, EEG, video telemetry, PET, or SPECT scans to pinpoint where seizures start in the brain. The most common type of surgery is resective surgery, where a small section of the brain—often in the temporal lobe—is removed. Other advanced procedures include LITT (Laser Interstitial Thermal Therapy), Deep Brain Stimulation, Corpus Callosotomy, and Hemispherectomy. The surgery is done under general anesthesia. The surgeon creates a small bone window and removes the seizure-generating brain tissue using electrocorticography (ECoG) guidance. Complete removal of the epileptogenic focus offers the best outcomes. The bone flap is then replaced and secured for healing. Hospital stay is usually 3–4 days, while full recovery may take 1–3 months. Fatigue is common in the early days, and most swelling and discomfort settle within a few weeks.
