But does that make things safer? This episode explores a study from a NHS hospital that compared 'standard' Root Cause Analysis investigations against a new approach called "Learning Teams."
Instead of just looking at what went wrong in a single moment, these teams bring in the people who do the work every day to find the flaws in the system, and to focus on improving the things that make for better, safer, and more efficient work. In theory.
Based on this sample, Learning Teams found twice as many ways to improve, and most of those changes fixed the environment rather than just blaming the person.
