Any person who uses J status to pursue graduate medical education in the US is automatically subject to a requirement that they return to their home country for at least 2 years.
In reality, it is relatively easy to qualify for an exception to this requirement.
Most IMGs and Canadian physicians who pursue medical residencies and fellowships in the US use J visas.
During the final year of their training, they find an employer in a medically-underserved areas who sponsors them for a J waiver, usually through a state Conrad 30 program. See http://shusterman.com/jwaiversfordoct...

After they receive their waiver, their employer sponsors them for an H-1B visa, and ultimately, for a green card.
