When assessing a patient's shoulder, the Apley Scratch Test allows you to quickly gather information on your patient's functional capacity, as it combines the movements of medial rotation with extension and adduction, and lateral rotation with flexion and abduction.

For the Apley Scratch Test, the patient will try to bring the hands together on the back, with one hand coming from above and the other from below. The patient should then repeat the movement with switched arms while you check for differences. It is often observed that the dominant side shows more restriction than the non-dominant side.

If you see a restriction, don't be fooled into thinking that the problem necessarily lies in the glenohumeral joint, as shoulder movement also involves the clavicle and scapula. Many shoulder problems are caused by insufficient scapular muscle control.
