UPJ Obstruction in Left Ureter | Hydronephrosis & Bladder Dysfunction | Ultrasound Case Study
Clinical History:
• Complains of urinary incontinence.
Sonographic Findings:
Left Kidney:
• Enlarged with moderate-to-severe hydronephrosis.
• Marked dilation of the renal pelvis and calyces noted.
• Cortical thickness appears reduced, suggesting chronic obstruction.
• No evidence of renal calculi or mass.
Left Ureter:
• Dilated proximal ureter, measuring 5.8 cm in length and 9.2 mm in diameter.
• Evidence of narrowing at the ureteropelvic junction (UPJ), consistent with UPJ obstruction.
Right Kidney:
• Normal in size, shape, and echotexture.
• No hydronephrosis or structural abnormalities.
Bladder:
• Small capacity bladder with reduced Maximum Cystometric Capacity (MCC: 105 mL).
• Post-void residual volume (PVR: 95 mL) indicates significant incomplete bladder emptying.
• Mild bladder wall thickening noted, with no diverticula or masses visualized.
IMPRESSION:
1. UPJ obstruction in the left ureter, leading to significant hydronephrosis in the left kidney and proximal ureteral dilation.
2. Reduced cortical thickness in the left kidney, raising concern for chronic obstructive changes.
3. Bladder dysfunction with reduced capacity and high post-void residual volume, possibly secondary to chronic obstruction or functional bladder issues.
Recommendations:
1. Urological consultation for evaluation of the UPJ obstruction and bladder dysfunction.
