Structural Heart Intervention Series: TAVR for Small Annulus - (Sachin Goel, MD)

Описание: This talk features a case of transcatheter aortic valve replacement (TAVR) in a high surgical risk patient with severe aortic stenosis (AS) with small aortic annular dimensions and low left coronary height. Data on differences in hemodynamics with TAVR with balloon expandable versus self-expandable supra-annular transcatheter heart valve are discussed. Dr Goel discusses the ongoing randomized SMART Trial in patients with severe AS and small annulus. Dr Goel discusses the case of small annulus in patient with severe AS and low left coronary height, successfully treated with the Self expanding supra-annular Evolut valve, with the technique of left coronary artery protection.

Objectives:
1. Understand differences in hemodynamics with TAVR with balloon expandable versus self-expandable supra-annular transcatheter heart valve.
2. Review the ongoing SMART randomized trial for TAVR in patients with small aortic annular size.
3. Review a case of severe AS in a patient with small aortic annulus at risk of left coronary obstruction, treated successfully with self-expandable transcatheter heart valve with the technique of left coronary artery protection.
