Rocktek ADR-01 Super Delay Analog Delay Pedal. Pedal can self-oscillate like mad. Does a great on the edge of oscillation delay where it almost drops over the cliff in feedback chaos. Lots of fun.
Opened it up and found out it uses the Panasonic MN3205 chip -- same as the Boss DM-3, Ibanez AD9 and Arion SAD-1.
