2 decades of customized ablations: Broad beam to RayTracing, Beijing Dec 9th, 2023: Kanellopoulos MD

Описание: This is my keynote presentation from Beijing China on Dec 2023 focused on my personal journey and over 100 peer reviewed publications and thousands of major meeting presentations later: Customized excimer laser ablations,-designed originally to treat irregular eyes, in lieu of corneal optics-have given us key clinical solutions in regaining cruvial visual function without the need of conreal transplantation, but have also “taught” us significant lessons in key elements of visual function: angle kappa and how it transforms a regular cornea, into a “tilted” prism for some patients, producing coma and other high order aberrations such as trefoil
Laser vision correction for myopia by definition interferes with spherical aberration and thus affects traditionally contrast sensitivity especially in mesopoc and scotopic conditions.
We have shown that regularizing irregular corneas such as in keratoconus, cornea scar, older decwntwred ablations etc. impoves UDVA but mainly provides gain in lines of vision in CDVA.
By applying this knowledge in naive myopic eyes and when studying carefully corneal diagnostics and angle kappa through topography-guided ablations we introduced the concept of TMR ( topography-modified refraction) used instead of the standard subjective manifest refraction for each patient.
TMR in topography-guided (Contura) treatments was introduced in 2015 and reported in the literature in 2016 by our team in Athens, Greece and “ignited” a new era in refractive surgery-the one seeking improvement of “whole-eye” optics along with spectacle independence and oir global effort to teach this through our Topo-guided University all day courses. The large number of corneal diagnostics and manual calculations has now been affirmed and essentially replaced by ray-tracing customization!
