This video is about love and life; it represents the hero's journey for Jack. Its also a tribute to the characters in Lost.
The lyrics of the song express that we can't fall in love alone, and ask who will be there to hold our hand when there are obstacles in our path. Jack has the love of Kate, as well as the love of his friends that guide him to the eventual realization of the afterlife.
The special people in our lives are never truly forgotten and the bonds that tie us are unbreakable. Even if its not recognized at first, the connection goes deeper than the conscious mind and provides enlightenment.
