This is Mo, a homeless disabled man in Newport, Isle of Wight.
He is wheelchair-bound, with swollen feet and hands, and has been in hospital twice in a coma.
Despite his condition, the council and NHS discharged him back to the streets with no help.
Mo could die on the street at any moment — yet nobody is helping him.
This is a clear failure of duty of care under the Care Act 2014.
