Suncoast PACE (Program of All-Inclusive Care for the Elderly) is a member of Empath Health, the country’s largest not-for-profit health system delivering non-acute care and services through its skilled, medical and holistic programs to individuals with chronic, advanced and terminal illnesses or experiencing grief throughout North Central, West Central and Southwest Florida.
We aim to fully support and care for Pinellas County seniors 55+ achieve a healthy quality of life, stay active and socially connected and live safely and independently in their homes and in the community. Additionally, we provide education, guidance and relief for their caregivers.
Our services include primary care, therapies, skilled care, social and emotional support and day care at our Suncoast PACE Day Center and Medical Clinic; in-home care and support; and transportation to and from Suncoast PACE or other PACE-approved specialists.
