In this Journal Club session, we present and discuss a systematic review and meta-analysis comparing Bone Marrow Aspirate Concentrate (BMAC) versus Platelet-Rich Plasma (PRP) for knee osteoarthritis. The paper was published in the Indian Journal of Pain (Volume 39) on 9 December 2025 and followed PRISMA 2020 methodology with PROSPERO registration.
What you will learn in this session:
Why orthobiologics are increasingly used in knee OA
Key biological mechanisms: PRP growth factors vs BMAC cellular + cytokine effects
Study selection (PRISMA flow), included studies, outcomes and follow-up points
Primary outcome: Pain reduction (VAS/KOOS pain components)
Secondary outcomes: Function improvement (WOMAC, IKDC)
Risk of bias assessment (RoB2, Newcastle–Ottawa Scale)
Clinical implications: when PRP is enough vs when BMAC may be preferred
Key takeaways from the meta-analysis discussion
Pain relief: BMAC and PRP showed comparable pain reduction at 1, 3, 6, 9, and 12 months.
Function: BMAC showed modest superiority in IKDC at several time points (notably 3, 6, and 12 months).
Safety: Both therapies demonstrated excellent safety profiles, mostly minor transient discomfort/swelling.
Heterogeneity: Significant variability existed across studies; sensitivity analysis clarified some functional advantages of BMAC.
Additional clinical discussion points covered
Why some patients respond poorly to PRP (inflammatory arthropathy overlap, nociplastic pain, selection factors)
Role of ultrasound guidance and injection plane considerations
PRP with high molecular weight hyaluronic acid: practical insights
Managing OA with effusion, NSAID timing, rehab and “prehabilitation” principles
Practical questions from participants (meniscal PRP, subchondral approaches, DMARDs, antiplatelets, etc.)
