If the patients sugar levels are not under control, if their sugar levels are going beyond 300 or 400 and their fasting sugar levels are less than 110 and post prandial levels are 140 or 160, we consider it as normal. When the sugar levels go beyond 300 and 400 and depending on the physician and the doctor then we can start insulin but early initiation of insulin is always better for better preservation of beta cells.
