Official Baseball Rule 5.07(a)(2) Comment states that with runner(s) on base, a pitcher is presumed to be in Set Position if the pitcher's pivot foot is in contact with and parallel to the pitcher's plate (pitching rubber) and the other foot in front of the pitcher's plate, unless the pitcher notifies the umpire of an intent to deliver from Windup Position.
Here, Quintana makes no such declaration of Windup Position, so the umpires are to presume Quintana is pitching from Set, due to the way Quintana engaged the rubber with his pivot foot parallel, etc. Thus, when Quintana takes a step back during delivery, as a pitcher ordinarily would do during Windup but not during Set, 3B Umpire Adam Hamari asked his crewmates whether Quintana had declared to any of them an intention to pitch from the Windup. With Quintana having failed to declare such an intention, Hamari called a balk for an illegal pitch from Set Position.
