To trigger this True Ending, acquire and consume three One Third of Umbilical Cord before defeating Gehrman.

This battle is more symbolic than challenging. By the time a player reaches this point, their skill is undeniable. Defeating the boss is merely a matter of a little more time and patience! The game's ending and the truth behind the story are the real focus!

To learn the truth of the game, you must obtain three "One Third of Umbilical Cord" items.
-Ending One (Submission): Consume no more than 2 One Third of Umbilical Cords. Then, talk to Gehrman and let him kill you. There is no final boss fight.
-Ending Two (Honorable Retirement): Consume no more than 2 One Third of Umbilical Cords. Then, talk to Gehrman and defeat him as the final boss.
-Ending Three (Childhood's Beginning): Collect and consume 3 or more One Third of Umbilical Cords. Then, talk to Gehrman and defeat him. This unlocks the Moon Presence as the true final boss.

You may need to play through the game multiple times to unlock all endings and discover the full truth.
