Intro is a mix of the 2017 live intro, in turn based of the live intro from 2007, and the 2004 intro from Reanimation.
Bridge is from the Reanimation song "1stp Klosr".
Extended outro is the song's outro from 2010 - 2015 (with no "dramatic pause").
Encore break is taken from the band's DSP (Digital Souvenir Package, aka official bootleg of shows) taken in Jakarta, Indonesia, in 2011.
