IDM Baby (Infant of Diabetic Mother) – First Feed Management
Why First Feed Is Important
IDM babies are at high risk of hypoglycemia due to excess insulin. Early feeding prevents low blood sugar.
First Feed Management
1. Timing ⏱️
Give first feed within 30 minutes–1 hour after birth
Do not delay feeding
2. Type of Feed 🍼
Mother’s breast milk (best choice)
If breastfeeding not possible:
Expressed breast milk
Formula feed (if EBM unavailable)
3. Amount of First Feed
5–10 ml/kg per feed (small, frequent feeds)
Feed every 2–3 hours
4. Blood Glucose Monitoring
Check blood sugar before 2nd feed (at 2 hours of life)
Normal blood glucose:
≥45 mg/dL
If low → manage immediately
5. If Baby Is Asymptomatic
Early oral feeding
Continue regular feeds
Repeat glucose monitoring
6. If Baby Is Symptomatic or Sugar Low 🚨
Symptoms:
Jitteriness
Poor feeding
Lethargy
Seizures
Management:
Give 10% dextrose bolus
👉 2 ml/kg IV
Start 10% dextrose infusion
NICU admission
7. Nursing Care
Maintain warmth
Observe for hypoglycemia signs
Strict intake–output chart
Support mother for breastfeeding
Key Point (Exam)
IDM baby should be fed early (within 1 hour) to prevent hypoglycemia. #newborncare #newbornbaby #babycare
