Keratoconus Diagnosis: Novel Insights from Corneal HOA Maps

Can corneal higher-order aberration (HOA) maps help diagnose and grade keratoconus?
This case series reveals how HOA excimer ablation map imaging can identify reproducible keratoconus features and correlate them to standard Pentacam indices.
In this study of 12 eyes with ≥ grade 2 keratoconus, Topolyzer Vario imaging revealed a consistent pattern of ablation map “islands” – including deep inferotemporal and shallow superonasal features – accompanied by superior peripheral crescents.
These unique patterns were:
• Highly reproducible in location and angular position
• Strongly correlated with posterior radius curvature (PRC) and Belin/Ambrosio enhanced ectasia total deviation (BAD-D) indices
• Coincident with the topographical apical cone location
Key takeaway:
HOA ablation maps may serve as a new imaging modality to aid early keratoconus detection, assess severity, and complement conventional corneal tomography techniques.
