About Dr. Schwartz: Dr. Jeremy Schwartz is an associate professor at Yale specializing in general internal medicine and chronic disease epidemiology. A Cornell graduate, he earned his MD from the University of Rochester and trained in Yale’s Internal Medicine and Pediatrics program. He serves as Firm Chief at Yale New Haven Hospital and Core Faculty for Yale’s Primary Care Program. Dr. Schwartz directs the Faculty Network Program at the Yale Institute for Global Health and co-leads chronic disease research in Uganda, focusing on addressing care gaps in resource-limited settings. He also leads the Global Chronic Care Access Lab at Yale.
