In a nutshell, a health and welfare power of attorney enables a nominated family member or friend to make decisions on your behalf regarding your day-to-day care and wellbeing.

choosing a care home or home care provider to look after you
deciding what your medical care should look like if you are unable to make the choices yourself
what any palliative care might look like
