The Association of Foot Arch Posture and Prior History of Shoulder or Elbow Surgery in Elite-Level Baseball Pitchers
1. Feigenbaum LA, Roach KE, Kaplan LD, Lesniak B, Cunningham S. The association of foot arch posture and prior history of shoulder or elbow surgery in elite-level baseball pitchers. J Orthop Sports Phys Ther. 2013;43(11):814-820. doi:10.2519/jospt.2013.4504
My Takeaways:
1) Longitudinal arch angle appears to have a correlation to elbow and/or shoulder surgery in baseball pitchers. (Regardless of atypical presentation, although pes planus is worse than pes cavus)
2) Therapists, athletic trainers, and pitching coaches should consider atypical LAA when working with overhead athletes for better performance and injury prevention.
3) Our body is connected as one system, don’t ignore other systems that may be at play!
Payton’s Takeaways:
1) The first sentence really hit me: "In Major League Baseball pitchers, injuries to the shoulder (28%) and elbow (22%) account for more days lost from play than any other anatomic areas, based on time on the disabled list."
2) Normal LAA is between 130-150 degrees
3) Athletes with a pes planus foot posture on their stance foot had an odds ratio of 3.7% and was found to be statically significant. This finding suggests that pes planus on the stance foot may alter loading during the wind-up phase of the pitching motion more than any other condition. This foot is flexible and unstable causing the athlete to have to compensate for stability in other areas-ie the shoulder
4) Pes cavus-not statistically significant: A result of increased leg stiffness is a decrease in sagittal plane excursion at the knee, reduced center of mass, larger vertical loading rates, and different muscle activity. This can limit the body's ability to decelerate through the pitching motion.
