-For Unvaccinated or Partially Vaccinated Individuals: RT-PCR Negative Result Certificate with date of extraction within 72 hours to date of travel
3. Confirmed DOT-Accredited accommodation
4. Roundtrip Flight Tickets
5. AkQuire OHDC QR Code - auto-generated via https://touristboracay.com/health-dec... - You can apply within 3 days from arrival date.
!! Keep a screenshot of the OHDC QR Code on your phone as it will be required at the airport and ports !!
