Extension or flexion of the fetal spine can determine how a baby's head comes into the opening of the pelvic floor. Unlike the bowl shape we were once taught, the pelvic floor is more funnel shaped, and similar to the shape of a Calla Lily. Gail mentions "right obliquity of the uterus" which is more than the womb leaning towards the right. Right obliquity shapes the uterus so that the right side is steeper, and the left side of the uterus is more curved. Therefore, a baby may be in extension when entering from the right, and more flexed when entering from the left.
