I am spoiled that I have ct scanner in the ed easily accessible. I skipped the Xrays to confirm what I suspected on my clinical exam.
Also I can make a disposition by reading my own xrays and ct scan imaging as an overnight EM attending for many years.
I am fortunate not doing the barbaric injection of the Saline Joint Injection of at least 50 ml and then close to 195 ml of larger joints like the knee to confirm an Open Joint Injury/Traumatic Arthrotomy.
Of course, after the official reads by radiology many hours later to confirm the diagnosis, Ortho consult asks for x-rays to be done.
Cefazolin 2 grams iv given already given, pre-op labs ordered, the patient kept NPO and admission for knee washout to prevent infection and make sure no gravel in that joint space. Washout tomorrow morning after midnight.
