The vast majority of people want to remain in their homes as they age, and in most cases, they know what they need to do so. CAPABLE is client-directed program that lets individuals set and achieve their goals for functional safety and independence at home. More than a decade of research shows that CAPABLE reduces the impact of functional impairment and disability, enhances motivation and self-efficacy, reduces health disparities, improves emotional health, and reduces hospitalizations and nursing home days. In this video, you'll hear from graduates of the program about how CAPABLE has changed their lives for the better.
CAPABLE is a program created by Johns Hopkins University School of Nursing and funded by St. David’s Foundation in Central Texas. Implementing partners include Meals on Wheels Central Texas and the Area Agency on Aging of the Capital Area.
