Hemoglobin is a tetrameric protein composed of globin chains and heme groups, and its structure varies across developmental stages. The major types of hemoglobin include HbA (α2β2), which is the predominant adult hemoglobin; HbA2 (α2δ2), a minor adult form; and HbF (α2γ2), fetal hemoglobin, which has a higher affinity for oxygen. In alpha thalassemia, reduced or absent alpha globin chain production due to deletions or mutations in the HBA1 and HBA2 genes disrupts the formation of normal hemoglobin types, leading to excess beta or gamma chains and the formation of abnormal hemoglobins such as HbH (β4) and Hb Bart’s (γ4). These abnormal hemoglobin variants contribute to ineffective erythropoiesis, hemolysis, and microcytic hypochromic anemia. Understanding the different types of hemoglobin, including normal hemoglobin variants and abnormal hemoglobin forms, is essential for interpreting hemoglobin electrophoresis, diagnosing alpha thalassemia, differentiating alpha thalassemia from beta thalassemia, and evaluating inherited blood disorders in clinical hematology.
