Question: Are there any red flags nursing home administrators should watch for when evaluating their wound care providers?
Dr. Mikhail Novikov: One of the most important things to look for is whether a provider or company is truly quality-based. A reasonable question to ask is: What is your incentive system for providers? How do you structure bonuses, if at all?
If you hear that they operate on an RVU bonus model—common in large organizations—understand that RVU is essentially a sales commission. It means the more procedures a provider performs, the more they earn. In my opinion, and as a company, we believe that’s the wrong approach. Incentives should be tied to making a difference for each patient: healing wounds when possible, preventing hospitalizations, and preserving quality of life.
In some cases, full healing may not be realistic—such as with a 95-year-old patient—but we can still aim to keep them out of the hospital, prevent infections, and allow them to remain in their familiar environment, participating in activities with their peers. That’s real quality care.
